BMI explained: what it measures and what it misses

Body Mass Index (BMI) is one of the most widely used health screening tools in the world — and also one of the most misunderstood. Knowing what it actually measures, and more importantly what it does not, helps you use it as a useful data point rather than a verdict.

What BMI actually is

BMI is a simple ratio of weight to height, calculated as:

BMI = weight (kg) ÷ height (m)²

It was developed in the 1830s by Adolphe Quetelet, a Belgian mathematician, as a way to describe the “average man” for population statistics — not as a clinical diagnostic tool for individuals. Its modern use in healthcare came much later, largely because it is cheap, fast, and requires no special equipment beyond a scale and a tape measure.

How BMI is actually used in the real world

Understanding where BMI shows up in practice explains both its persistence and its controversy. In clinical settings it is a two-second screening step: an out-of-range value doesn't diagnose anything, but it prompts the conversation and the follow-up measurements that can. In research and public health, BMI is invaluable precisely because it's cheap and universal — tracking obesity trends across millions of people is only possible with a metric anyone can compute from two numbers.

Where it gets more contentious is in administrative use: some insurers factor BMI into premiums, some surgical guidelines use BMI cut-offs for eligibility, and some fitness standards lean on it — applications that treat a population-level screen as an individual verdict, which is exactly what it was never designed to be.

It's also worth knowing the categories themselves are somewhat historical artefacts: the familiar 18.5/25/30 cut-offs were standardised by the WHO based on mortality-risk data, and several countries apply lower thresholds for populations where health risks rise at lower BMIs. The number is a convention — a useful one — not a law of nature.

My BMI is normal but my waist is large — should I worry?

Possibly, yes — central fat carries more metabolic risk than BMI captures. A waist-to-height ratio above 0.5 is a common flag even at normal BMI, and worth raising with a doctor.

Does BMI apply during pregnancy?

No. Standard BMI categories aren't meaningful during pregnancy; clinicians instead use pre-pregnancy BMI to guide recommended weight-gain ranges. Use your pre-pregnancy figures if you want a baseline.

Is BMI reliable for older adults?

Less so. Muscle loss with age means an older adult can hold a 'normal' BMI while carrying relatively high body fat, and slightly higher BMIs are associated with better outcomes in some elderly populations. Composition measures add a lot of value here.
